Caring Partners Coalition
The Caring Partners Coalition is a network of agencies that provide services to caregivers and their families. The Coalition members work together to create an awareness of caregiver issues and community resources through outreach and education and to advocate on behalf of caregivers and those to whom they provide care. Service Description: Programs include information, referral, support and education for family and professional caregivers of Alzheimer's patients. Description: Delaware Hospice has been providing exceptional care and comfort since 1982, and is the largest, most experienced and only nonprofit hospice in our area. It is composed of a team of professionals and certified volunteers. It is dedicated to enhancing quality of life, so patients and their families can find joy in each and every day.
Payment: Families are never billed for services and costs are covered by Medicare, Medicaid, private insurance or contributions given by the community. No one is ever denied care based on ethnicity, age, sex, mental or physical status, or religion.

Service Description: This state division helps older Delawareans and persons with physical disabilities improve their quality of life and lead active and productive lives, with the greatest level of independence. DSAAPD provides a broad range of direct consumer services that are listed on its website. One of the programs sponsored by DSAAPD is CARE Delaware, an outreach to family caregivers.

Service Description: The agency links working parents and other employees with services that help them balance their work and life responsibilities, and to improve the supply and quality of these services in communities where employees live and work. Services include counseling, education, and referral services for caregivers, respite and visitation programs for older Delawareans, and education and advocacy for the general community.
Fee: Access Care Dependent Care Resource & Referral Services free to the public. Various levels of enhanced resource & referral services are available for a nominal fee. Services provided as an employee benefit are paid for by the employer.

Service Description: The center provides the environment and program opportunities for adults over 50 to come together in a social setting to explore education and enrichment opportunities, cards and games, day and extended trips, wellness and fitness activities including a supervised fitness center and therapeutic pool. Adult volunteers are incorporated into the paid staff to provide a rich environment of many interests. Other programs include support groups for caregivers and Meals on Wheels.

Service Description: An interdisciplinary team provides comprehensive geriatric assessment including longitudinal cognitive assessment, functional assessment, residential assessment, family assessment, psychosocial assessment, and complete history and physical exam. Recommendations for the family and client are provided in a report that is also available to the primary care provider. Referrals for service can be generated by family members, friends, or members of the health care team.
